Click4Corp supports a drug-free workplace under the Drug Free Workplace Act. All employees are subject to drug testing under the following circumstances: Pre-Employment, Post-Accident, Random, and Reasonable Suspicion.
The full policy prohibits the unlawful manufacture, use, sale, transfer, distribution, dispensation, or possession of alcohol, drugs, or controlled substances at any Click4Corp work site, including company vehicles and private vehicles parked on company premises.
"Under the influence" means being unable to perform work safely and productively, or having any detectable level of alcohol, drugs, or controlled substances in the body. Violations result in discipline up to and including termination.
Urine, breath, blood, hair, and/or saliva samples may be collected. Refusal to submit or altering a sample is cause for immediate termination. The company reserves the right to search employees and their personal effects on reasonable suspicion.
Pre-employment testing applies to all new hires. Reasonable-cause testing requires a supervisor witness and documentation within 24 hours. Post-accident testing must occur within 32 hours. Random testing is conducted throughout the year.

You may elect to retain your common-law right of action if, no later than five days after you begin employment (or within five days after receiving written notice from the employer that the employer has obtained coverage), you notify your employer in writing that you wish to retain your common-law right of action. If you elect to retain this right, you cannot obtain workers' compensation income or medical benefits if you are injured.

If injured on the job in a service area: you must choose a treating doctor from the network list, or ask your HMO primary care physician to serve as your treating doctor (notify Texas Mutual at 800-859-5995). You must go to your treating doctor for all care; referrals are for specialists; emergency care can be obtained anywhere. The insurance carrier pays in-network providers. You may be responsible for bills incurred out of network without approval. False or fraudulent claims are a crime.
Call (800) 381-8067 if you need a network treating provider. Do NOT return this form to Texas Mutual unless requested.
Should I become injured while performing duties within the scope of my employment with Click4Corp, I understand my responsibility to comply with the following. Non-compliance may result in a written warning and possible termination:
For good consideration and as an inducement for CLICK 4 CORP LLC to employ the undersigned, Employee agrees not to directly or indirectly compete with the business of the Company and its successors and assigns during the period of employment and for a period of 5 years following termination of employment, regardless of cause. "Not compete" means Employee shall not own, manage, operate, consult, or be employed in a business substantially similar to or competitive with the present business of the Company.
Employee acknowledges the Company may provide access to trade secrets, clients, and other confidential data. Employee agrees to retain this information as confidential and not to use it or disclose it to any third party. This agreement binds and inures to the benefit of the parties, their successors, assigns, and personal representatives.
This Confidentiality Agreement between Click 4 Corp and the undersigned covers: (1) the purpose of discussing mutually beneficial transactions requiring disclosure of business, financial, and technical information; (2) a definition of Confidential Information that excludes public-domain information; (3) a commitment to take reasonable measures to protect confidential information; (4) an "as is" warranty on information provided; (5) a term of three (3) years from the effective date unless renewed in writing; (6) remedies including injunctive relief, with arbitration in Dallas, Texas; and (7) miscellaneous terms governed by Texas law.
